If your favorite dentist is not on our list, when you become a member, you receive a dentist referral card, which you can fill out and mail in, and the Plan will contact your dentist to become a provider if they so elect.

All pre-existing conditions are covered except orthodontic treatment in progress. There are no deductibles, no claim forms to fill out and no limits on visits to your dentist.
